Alcohol Establishment Licenses

Alcohol Establishment Licenses are now online through OpenGov!

Moving forward, once you create an account with OpenGov, you can apply for multiple licenses through the City each year, using the same account (including Dog License/Operator’s License/Special Event/etc).
Your personal OpenGov account will allow you to:
- Submit a new/renewal Alcohol Establishment License Application
- Make a payment with an online payment option
- Track the progress of your Licenses
- Message the City with questions or comments regarding your application
- Print your Establishment Licenses (THE CLERK’S OFFICE WILL ONLY PRINT UPON REQUEST). Once approvals have been completed, the system will notify you when printing is available. All applicants have the ability to print at home.
- Estimated processing time is 6 weeks
The License period is from July 1st through June 30th. All Licenses expire June 30th. License MUST be Displayed at Business
Renewal Licenses must be applied for by April 15th or be subject to a late filing fee of $250.00 per Ordinance 60-40(11)
Renewals for Liquor Licenses in OpenGov:
For those applicants that have applied previously through the OpenGov portal, you'll need to reapply using the Renewal functionality.
Important Instructions for Alcohol License applicants:
DOR Retail Alcohol Beverage License Applications and Miscellaneous Forms
Your application cannot be accepted without all required information and documents.
You will need the following information/documents to complete your application:
- A copy of your CURRENT Wisconsin Seller's Permit
- Your Federal Employer Identification Number (FEIN)
- An AB-100 Alcohol Beverage Individual Questionnaire for Owners of the Alcohol Establishments (One for each person listed on the application (if applicable)).
- Please print off one form for each owner here: AB-100 Alcohol Beverage Individual Questionnaire
- A copy of a Driver's License/State-Issued ID for each person listed on the application.
- Forms for your appointed Agent:
- AB-100 Alcohol Beverage Individual Questionnaire form for the Liquor Agent for corporations/nonprofit organizations/limited liability companies (LLC) (if applicable): AB-100 Alcohol Beverage Individual Questionnaire
- A copy of a Driver's License/State-Issued ID for the Liquor Agent.
- AB-101 Alcohol Beverage Appointment of Agent form: AB-101 Alcohol Beverage Appointment of Agent
- Appointment of Liquor/Beer Agent form: Appointment of Liquor/Beer Agent
- Agent Authorization Letter form: Agent Authorization Letter
- A complete description of the licensed premises. This should describe the building or buildings where alcoholic beverages are to be sold and stored. The applicant must include all rooms including living quarters, if used, for the sales, service, consumption, and/or storage of alcohol beverages and records.
- A scaled floorplan.
- A security/operational plan.
*Remember: You need to print one (1) copy of the AB-100 Auxiliary Questionnaire form for each person listed on the application and:
- Have each person complete and sign the form.
- Then scan both the form and a copy of their driver's license or state-issued ID and attach them to the online application.

If you are not renewing for the 2026-2027 Licensing year, please notify the City Clerk’s Department immediately.
Operator's License (a.k.a. Bartender's License)

- Print your Operator's Licenses (THE CLERK’S OFFICE WILL ONLY PRINT UPON REQUEST). Once approvals have been completed, the system will notify you when printing is available. All applicants have the ability to print at home.
- Estimated processing time is 1-2 weeks
Operator Licenses are a two year term, expiring June 30th of every even year regardless of when applied for. License MUST be displayed at Business working for.
The License fee is $70.00. Fees are due at the time of Application
Review of your application:
- The Fitchburg Police Department will perform a background check to verify that the
information you have provided is complete and accurate.
- If there are concerns about your arrest and/or conviction record as it relates to your application,
or if it appears that you falsified or omitted information from your application, you may be called to appear before the Public Saftey and Human Services Committee (PSHS).
- If you are asked to appear but choose not to do so, your application may be denied.
- Meetings of the Public saftey and Human Services Committee Meetings are open to the public and televised.
